Sign up for a 30 minute online health check with Chest Heart and Stroke.
The session is client led so the information and support provided will be based on what you as the client wants to gain from the session and is closed by setting some small goals which the client can focus on.
Each client will receive a personalised report based on their lifestyle questionnaire including supporting information relevant to them.
